[Fix] Completed task fails to resume after restart when a message write is interrupted - #2136
safeWriteJson replaced a file with two renames: it renamed the target to a backup, then renamed the replacement into place. A process exit between the two renames left the canonical path absent. Readers then saw a permanent ENOENT, and no exception rollback could run after process termination. This reproduced 5/5 with subprocess fault injection in the issue. It is the storage failure behind the restart-persistence CI failure in PR #2092.
Writer
The writer puts the new content in a temp file.
The writer renames the temp file over the target in one step. The committed target stays in place until the rename lands. An interrupted write can no longer remove the canonical path.
A recovery pass runs at the start of every write, under the advisory lock. If the canonical path is absent, it restores the newest .bak_*.tmp backup. It then removes leftover .new_*.tmp and .bak_*.tmp files.
The writer never deletes a backup it could not restore. If a restore or a rollback fails, the write stops and all backups stay on disk.
Optional write trace callbacks cannot throw into persistence. Completion tracing runs outside the rollback catch. A successful replacement is never rolled back for a diagnostic failure.
Readers
If the canonical file is missing, readers retry once. After the retry, they run the same recovery under the lock and read again. The first read of an existing file takes no lock.
ui_messages, API history, task history items, and ClineProvider.getTaskWithId use this path. getTaskWithId returned an empty API history before this change. The real-window restart E2E found this.
Only SyntaxError and ENOENT map to "no data" in the shared history readers. Reconciliation also treats ENOTDIR as an absent task record. Other read errors propagate.
Bulk state publication uses the store only when it is healthy. Otherwise, it uses legacy global-state history, which can contain stale entries. Disabling history still omits it. Successful provider initialization refreshes an open webview.
Deleting a task removes its backups under the same lock, so a deleted task cannot come back from a backup.
Startup
TaskHistoryStore.reconcile() now recovers a missing history_item.json before it treats the task as gone. Before this change, a task with an orphaned backup left the cache. Delegation repair then read "child not found" and cleared delegation links.
Forced startup and replay scans finish reading other tasks, then reject if any task was unreadable. Delegation repair cannot use that partial snapshot.
Background scans retain unreadable cached records and continue updating readable tasks. They log the first error and still evict records confirmed absent.
initialize() retries ELOCKED and EBUSY with delays of 1, 2, 4, 8, 8, and 8 seconds. Concurrent calls share one promise. dispose() cancels a pending retry.
initialized resolves after success or after terminal failure. isHealthy is true only after a full startup. Extension activation still continues after a terminal failure. Then history can stay incomplete until the next window start.
Observability and CI
saveTaskMessages emits trace points with the task ID and process ID. hasTaskUiMessageText traces the marker check. deactivate traces shutdown. Every failed recovery emits json-recovery-failed with the path and the error code. One sink in src/utils/persistenceTrace.ts serves all of these. The extension mirrors it into the output channel.
runTest.ts writes metadata for a failed restart scenario, then removes its temporary workspace. Entries classify backups, new temp files, and locks. Hashed directory IDs group entries without exposing task IDs. Trace entries contain only a validated stage, PID, and timestamp. Raw histories, logs, phase values, error text, and arbitrary filenames remain excluded. The workflow uploads only diagnostics.json, with seven-day retention.
Docs
docs/architecture/json-persistence-audit.md records the write contract, the callers, and the gaps below.
Decisions to check
A fresh lock left by a killed process (SIGKILL) makes writers and recovering readers fail with ELOCKED until the lock goes stale (31 seconds). Lock age cannot tell a dead process from a paused one. A test covers this, and the behavior is deliberate.
The recovery pass removes matching temp files while it holds the lock. This relies on the advisory-lock model of safeWriteJson.
The old fsync step is gone. The replacement protects against process termination. It does not protect against power loss.
Test Procedure
Deterministic fault injection:
pnpm json-replacement:model-check checks 87 production filesystem cutpoints with 522 crash and I/O fault cases. Each case gets isolated read, write, delete, and cold-start follow-ups. A four-worker pool preserves coverage and reduces serial runtime. JSON_REPLACEMENT_WORKERS accepts values from 1 to 8. Three negative controls must detect backup loss, resurrection after deletion, and reader overwrite.
safeWriteJson.interruption.test.ts starts a real Node child process with this repository's safeWriteJson. The child exits before the rename that targets the canonical path. The test then reads the file and requires the previous content. A recovery write must succeed and remove the leftovers. On the old code, the read fails with ENOENT 3/3.
A second variant uses SIGKILL, not process.exit. It checks that a fresh lock rejects a writer and a reader with ELOCKED. It checks that the backup stays. It ages the lock past 31 seconds and checks that recovery works.
Unit tests cover the single-rename commit, the EPERM fallback, a failed restore, a failed rollback, backups kept after a failure, delete without resurrection, reads that recover, and the trace sequences.
TaskHistoryStore.recovery.spec.ts and TaskHistoryStore.initialization.spec.ts cover startup: orphaned backups, failed restores, fresh and stale locks, retry delays, the cap, give-up, and disposal.
The restart-persistence E2E moves the UI and API histories to orphaned backups in a fresh extension host. It requires the public reads to restore the exact bytes. The E2E runs in real VS Code windows on Linux.
Results on the working tree:
The current focused persistence selection passes: 18 files, 499 tests. Five metadata-artifact tests and seven workflow-contract tests pass. Source types, changed-file lint, and formatting pass.
The compile job now runs pnpm --filter @roo-code/vscode-e2e test:restart-artifacts on pull requests. The filesystem-only privacy tests run independently of the extension-host E2E cache.
Earlier author runs reported passing types, lint, formatting, model checks, workspace tests, and restart E2E. Those results predate the crash-cutpoint checker.
The four-worker crash campaign takes about 45 seconds on the development machine (42 to 45 seconds measured), with all 87 cutpoints and 522 cases. Focused script types and formatting pass.
The project check, pnpm --dir src run check-types, passes. CI runs that check through Turbo.
The stricter root tsc --noEmit -p . still reports 468 diagnostics, matching main (3b3f6c780fb8b9ea452d277edfdf58d08284dfa5) with the same installed dependencies. The comparison normalizes paths and ignores source line numbers. The extra safeWriteJson diagnostic is fixed: recovery now checks the first backup before it reads the path. The root command is not CI's project type check.
The current full workspace run passes all 14 tasks (12 cached). Command: VITEST_MAX_WORKERS=2 pnpm test --concurrency=1 --env-mode=loose. The extension suite passes 11,341 tests, with 39 skipped.
The current restart E2E passes in real VS Code windows on Linux. An injected runner failure preserves a sanitized manifest with leftovers and traces from both extension hosts. The workflow upload glob matches the manifest locally. GitHub artifact upload and native Windows behavior remain unverified.

I saw no measurable change. The differences are smaller than the run-to-run noise. Each saved UI history emits three trace lines. This cost nothing visible with a silent sink or a stderr sink. I did not measure the output-channel sink. I did not measure Windows, slow disks, lock contention, or startup with thousands of tasks.
Known Limits and Follow-up
Bounded crash coverage. The checker enumerates crashes and faults at baseline call boundaries, including the preset EPERM/EIO rollback path. It does not explore concurrent actors or crash combinations with additional replay-injected faults beyond the fixed scenario failures. Recovery follow-ups run after leftover locks are aged. It does not prove power-loss durability or native Windows behavior.
Windows is not verified. The EPERM branch is fault-injected in unit and subprocess tests. No test ran on Windows. The fallback handles only EPERM. EBUSY or EACCES on Windows are not handled.
A paused or crashed peer can delay startup for up to the retry total. Then history stays partial until the next window start. The warning message and the manual retry action are not part of this PR.
Unresolved writers. These do not use the shared protocol: the McpHub read-modify-write, the SimpleInstaller direct writes, and the project MCP settings initialization in webviewMessageHandler.ts. The audit document lists them. They need their own issues.
Test order. Running ClineProvider.spec.ts with --sequence.shuffle fails 40 to 51 tests in groups this PR does not touch. I have not run the same seed on main, so I call these failures unclassified. A new recovery test in that file did depend on test order. I fixed it.
Not a crash fix. This PR does not stop the extension process from crashing. It keeps committed data readable after an interruption.

Full details: Linked Issues check
Explanation
Issue #2130 is the only active direct link. The PR meets its coding requirements. safeWriteJson uses a single rename for normal commits and retains the EPERM fallback. Recovery restores missing canonical files from backups under the lock. Reader retry and recovery distinguish a transient ENOENT from an interrupted replacement. Subprocess, unit, model-check, and restart-E2E tests cover the replacement failure. The PR also preserves sanitized restart diagnostics and uploads them in CI. The reported Windows and power-loss limits do not contradict the issue requirements.
Full details: Out of Scope Changes check
Explanation
The changes stay within Issue #2130. TaskHistoryStore recovery, reconciliation, initialization handling, deletion cleanup, and provider read-path changes protect restart task history. Tracing, sanitized failure artifacts, workflow checks, tests, and architecture documentation support the issue's regression and diagnostic objectives. No changed area has a demonstrated unrelated purpose.
Full details: Security Boundaries
Explanation
No changed path meets the security failure condition. preserveScenarioWorkspace publishes only file metadata, hashes, allowlisted filenames, validated persistence-trace fields, and validated phase status; it does not copy task contents, logs, or errors. The artifact workflow uploads only diagnostics.json, and the added tests assert that secret-like content is absent. Persistence traces record task IDs, paths, stages, and error details, not message or API-key contents. The new checker executes a fixed child script and allowlists mutation, regression, and worker values; no changed code executes unvalidated input or bypasses an approval or allowlist control.
Full details: Persistence Integrity
Explanation
No changed persistence path meets a failure condition. safeWriteJson awaits directory setup, lock acquisition, temp streaming, commit, cleanup, and lock release. Its normal commit uses one awaited rename, and the Windows EPERM fallback has awaited rollback; failed rollback preserves the backup for later recovery (src/utils/safeWriteJson.ts:230-395). Readers await retry and recovery, and task-history writes, deletes, reconciliation, and initialization await their persistence operations. Changed consumers also await safeWriteJson, readFileWithMissingRetry, and recovery calls.
Full details: Regression Evidence
Explanation
The standing regression-evidence gaps remain. preserveScenarioWorkspace removes an existing destination at apps/vscode-e2e/src/restart/scenarioWorkspace.ts:184-186, but scenarioWorkspace.test.ts never pre-populates or verifies stale-destination removal. runTest.ts:233-245 adds distinct failed-preservation and successful-removal cleanup paths, but no focused test covers either path. API.hasTaskUiMessageText now emits match, no-match, and read-error trace events at src/extension/api.ts:365-375, while its focused tests assert only return values and reads. extension.ts:132 installs the dual persistence sink and extension.ts:410 emits shutdown, but src/__tests__/extension.spec.ts asserts neither integration. No durable visible UI change requires a Playwright snapshot.
Resolution
Add focused tests for stale destination removal and both runTest.ts cleanup outcomes, preferably through an extracted cleanup helper. Extend api-has-task-ui-message-text.spec.ts to assert trace stage, taskId, result, and error detail for match, no-match, and read-error cases. Extend src/__tests__/extension.spec.ts to assert dual-sink installation during activation and the shutdown trace during deactivation.
Full details: Lifecycle Resource Cleanup
Explanation
The changed trace lifecycle still retains a disposed output channel. activate() installs a module-level sink closure with setPersistenceTraceSink(createDualLogger(createOutputChannelLogger(outputChannel))) at src/extension.ts:127-132, while outputChannel is registered in context.subscriptions. deactivate() emits shutdown but never clears the sink at src/extension.ts:409-411. After disposal, a later persistence event can invoke the retained logger against the disposed channel. The trace function swallows the resulting error, so the stale resource and callback remain hidden. No lifecycle test covers post-deactivation trace events.
Resolution
In deactivate(), emit the shutdown trace first, then call setPersistenceTraceSink(undefined) so the global sink no longer retains the output channel. Add a lifecycle test that activates the extension, deactivates it, emits a later persistence event, and verifies that the disposed output channel is not called or retained.

Automated review is complete for the latest commit but does not replace human approval.
Review-state labels are managed by this workflow; do not edit them manually. community-approved is managed the same way — do not add or remove it manually. It signals a fresh community code approval for the current head as an advisory priority only; maintainer review is still required.
safeWriteJson replaced a file with two renames (target to backup, then
replacement to target). A process exit between the renames left the
canonical path absent, so readers saw a permanent ENOENT that no retry
could repair.
The replacement is now committed with a single atomic rename. Only a
Windows-style EPERM (target held open by a reader) falls back to the
two-step sequence, and a recovery pass under the advisory lock restores
the newest orphaned backup and removes leftover temp files before any
write. Task-message saves emit trace points with the task ID and
process ID, the marker check and shutdown are traced, and a failed
restart-persistence scenario now preserves phase results, VS Code logs,
and task files for CI artifact upload.

The root workspace declares esbuild, and Node resolves dependencies from the test directory through ancestor node_modules directories. The repository already uses the same root dependency from src/esbuild.mjs. Knip also explicitly ignores esbuild as an unlisted dependency.

- Line 443: Update the per-task scan in reconcile() so a non-absent read error
keeps the task ID in liveIds, records the first scan error, and continues
scanning; this prevents eviction of the unreadable task and lets other tasks be
processed. After the eviction loop, rethrow the recorded error only when
options.forceRefresh is set.

## Task Lifecycle Changes
- Add extension-host E2E coverage only for a boundary the reducer model cannot prove, such as restart visibility, real persistence/rehydration, delayed provider streams, scheduler permits, or webview task scoping. Do not duplicate reducer interleavings in E2E.

Closes: #2130
Description
safeWriteJsonreplaced a file with two renames: it renamed the target to a backup, then renamed the replacement into place. A process exit between the two renames left the canonical path absent. Readers then saw a permanentENOENT, and no exception rollback could run after process termination. This reproduced 5/5 with subprocess fault injection in the issue. It is the storage failure behind the restart-persistence CI failure in PR #2092.Writer
EPERM. The two-step replacement from [Fix] Restart-persistence E2E flakes when the final ui_messages.json write lands after VS Code quits #2080 stays as the fallback for that case only..bak_*.tmpbackup. It then removes leftover.new_*.tmpand.bak_*.tmpfiles.Readers

The real-window restart E2E found this.SyntaxErrorandENOENTmap to "no data" in the shared history readers. Reconciliation also treatsENOTDIRas an absent task record. Other read errors propagate.Startup
TaskHistoryStore.reconcile()now recovers a missinghistory_item.jsonbefore it treats the task as gone. Before this change, a task with an orphaned backup left the cache. Delegation repair then read "child not found" and cleared delegation links.initialize()retriesELOCKEDandEBUSYwith delays of 1, 2, 4, 8, 8, and 8 seconds. Concurrent calls share one promise.dispose()cancels a pending retry.initializedresolves after success or after terminal failure.isHealthyis true only after a full startup. Extension activation still continues after a terminal failure. Then history can stay incomplete until the next window start.Observability and CI

Every failed recovery emitsjson-recovery-failedwith the path and the error code. One sink insrc/utils/persistenceTrace.tsserves all of these. The extension mirrors it into the output channel.runTest.tswrites metadata for a failed restart scenario, then removes its temporary workspace. Entries classify backups, new temp files, and locks. Hashed directory IDs group entries without exposing task IDs. Trace entries contain only a validated stage, PID, and timestamp. Raw histories, logs, phase values, error text, and arbitrary filenames remain excluded. The workflow uploads onlydiagnostics.json, with seven-day retention.Docs

pnpm json-replacement:model-checkchecks 87 production filesystem cutpoints with 522 crash and I/O fault cases. Each case gets isolated read, write, delete, and cold-start follow-ups. A four-worker pool preserves coverage and reduces serial runtime.JSON_REPLACEMENT_WORKERSaccepts values from 1 to 8. Three negative controls must detect backup loss, resurrection after deletion, and reader overwrite.safeWriteJson.interruption.test.tsstarts a real Node child process with this repository'ssafeWriteJson. The child exits before the rename that targets the canonical path. The test then reads the file and requires the previous content. A recovery write must succeed and remove the leftovers. On the old code, the read fails withENOENT3/3.A second variant uses
SIGKILL, notprocess.exit. It checks that a fresh lock rejects a writer and a reader withELOCKED. It checks that the backup stays. It ages the lock past 31 seconds and checks that recovery works.Unit tests cover the single-rename commit, the
EPERMfallback, a failed restore, a failed rollback, backups kept after a failure, delete without resurrection, reads that recover, and the trace sequences.TaskHistoryStore.recovery.spec.tsandTaskHistoryStore.initialization.spec.tscover startup: orphaned backups, failed restores, fresh and stale locks, retry delays, the cap, give-up, and disposal. The restart-persistence E2E moves the UI and API histories to orphaned backups in a fresh extension host. It requires the public reads to restore the exact bytes. The E2E runs in real VS Code windows on Linux.
Results on the working tree:
pnpm --filter @roo-code/vscode-e2e test:restart-artifactson pull requests. The filesystem-only privacy tests run independently of the extension-host E2E cache.pnpm --dir src run check-types, passes. CI runs that check through Turbo.tsc --noEmit -p .still reports 468 diagnostics, matching main (3b3f6c780fb8b9ea452d277edfdf58d08284dfa5) with the same installed dependencies. The comparison normalizes paths and ignores source line numbers. The extrasafeWriteJsondiagnostic is fixed: recovery now checks the first backup before it reads the path. The root command is not CI's project type check.VITEST_MAX_WORKERS=2 pnpm test --concurrency=1 --env-mode=loose. The extension suite passes 11,341 tests, with 39 skipped.Performance
I compared the merge base with this branch on one machine (WSL2, overlay filesystem). Each run used the real
safeWriteJson. Each cell shows the range of the median times from two runs.I saw no measurable change.
The
EPERMbranch is fault-injected in unit and subprocess tests. No test ran on Windows. The fallback handles onlyEPERM.EBUSYorEACCESon Windows are not handled.A paused or crashed peer can delay startup for up to the retry total. Then history stays partial until the next window start. The warning message and the manual retry action are not part of this PR.
Unresolved writers. These do not use the shared protocol: the McpHub read-modify-write, the
SimpleInstallerdirect writes, and the project MCP settings initialization inwebviewMessageHandler.ts. The audit document lists them. They need their own issues.Test order. Running
ClineProvider.spec.tswith--sequence.shufflefails 40 to 51 tests in groups this PR does not touch. I have not run the same seed onmain, so I call these failures unclassified. A new recovery test in that file did depend on test order. I fixed it.Not a crash fix. This PR does not stop the extension process from crashing. It keeps committed data readable after an interruption.
